I happened to have a portable breath carbon monoxide (CO) meter with me (the bar owner dealt in sleep-breathing equipment, and I had used that meter's readings many times). His baseline exhaled CO during his cigarette years was roughly 18–22 ppm; two weeks after quitting, fasting in the morning, it could drop to 4–6 ppm. That night at 21:40, he puffed it "mouth-only" for about 35 minutes, and the stub was still hot when he put it out. At 22:20 he measured again: exhaled CO was back around 14 ppm. Yet he said himself: "My throat doesn't burn, and my head feels less floaty — it feels cleaner than cigarettes."
What I kept wasn't his "cleaner," but that two things were true at once: it felt softer, and the biomarker had already rebounded. The rest of this article is really answering that pair of contradictions — **why cigars often "feel milder" even though exposure and risk are not correspondingly lower**. The differences hinge on three things: whether there's a filter, how it burns, and how you inhale.
No Filter: Not "Primitive Romance," Just a Shifted Site of Deposition
Most commercial cigarettes carry an acetate-cellulose filter; mainstream cigars have virtually no comparable filter structure. A cigar's wrapper is tobacco leaf and the inside is tobacco — the whole cigar is tobacco. Agencies such as the NCI wrote long ago: cigar smoke can carry higher concentrations of carcinogens, tar per unit of tobacco is often "dirtier," wrapper porosity is low, and combustion is more incomplete.
The filter itself is far from a "safety device" — it alters draw resistance, particle size, and pulmonary deposition, and historically it has triggered ventilation-hole-related compensatory smoking. But on the level of **immediate experience**, "filter or no filter" rewrites how your body feels at once:
- **Cigarettes**: smoke is "shaved" by the filter; some particles and condensate are trapped; draw resistance is relatively stable; the lung-draw hit comes fast; the throat has the familiar rasp.
- **Cigars**: smoke runs straight from the burning end through the tobacco column into the mouth. Without that acetate layer, the **mouth, nasal cavity, tongue surface, and soft palate** take the first hit of a stickier, oilier, more alkaline cloud. Many people read this "no throat rasp, no lung hit" as milder; I prefer to read it as — **the irritation has shifted forward from the lower respiratory tract to the upper digestive mucosa**.
In April 2022, at a cigar session in Guangzhou, someone used a knife to clip a beginner's cap and cut too deep; within less than ten minutes, tarry oil ran down the cigar and his fingers were sticky and brown. That was condensed tar-mixture, not some "tobacco-oil aroma." When I got home that night and looked in the mirror, the edges of my tongue were yellowed and the gaps between my teeth held a fermented tobacco smell that a night of brushing could not remove. That is the everyday form of the no-filter path: the toxicants did not "disappear," they just **get laid down first in the mouth and nasal cavity**.
In the public education materials of Taiwan's Health Promotion Administration (HPA), cigar combustion products can contain about 7,000 chemicals, of which about 93 are carcinogens; it repeatedly corrects the claim that "mouth-drawing is safe." The oral and nasal mucosae absorb nicotine quickly — cigar smoke tends to have higher pH, and the alkaline environment favors free-base nicotine crossing the mucosa. Not "pushing" the smoke into your lungs does not mean nicotine never enters your system.
My personal take is blunt: **no filter makes people underestimate the real load of a single session**. It "feels mild" because the familiar lung pungency is gone — but cotinine- and nitrosamine-related exposure does not score itself by how you feel.
Combustion and Temperature: Slower and More Incomplete Often Means Higher Concentrations of Dirty Stuff
"Burning temperature" is often treated as mysticism in bar talk. In practice, I care about three observable things: the cigar's diameter and length, the draw interval, whether the ash holds firm, and whether the sidestream smoke stings the eyes.
A single cigar usually holds far more tobacco than a single cigarette. A Robusto-class handmade cigar — many people smoke it for 45–90 minutes; a cigarette is usually done in 5–8 minutes. Once the time stretches, **time-weighted exposure is underestimated**: you think "I only had one today," but it was actually a nearly hour-long, low-frequency combustion session, sidestream and mainstream smoke alternating around your mouth and nose.
Packaging differences also matter. Cigarette paper is relatively porous with mature burn-engineering; cigar wrapper leaves have low porosity and combustion tends to be more incomplete. The direct consequence of incomplete combustion is that **tar, carbon monoxide, and several incompletely oxidized products** tend to be elevated in the smoke. Mayo Clinic's public Q&A is blunt: smoking cigars is not safer than smoking cigarettes; cigar smoke can carry more tar and can expose you to higher levels of carbon monoxide and other toxicants. The CDC likewise stresses: cigar smoke contains the same toxic and carcinogenic compounds as cigarette smoke, and it harms both smokers and bystanders.
Fermentation is another path often marketed as "craft aesthetics" but not romantic in toxicology. Fermented cigar tobacco can push tobacco-specific nitrosamines (TSNAs, e.g., NNN, NNK) to high levels; the HPA's public education explicitly notes that fermentation produces high concentrations of TSNAs, and with more incomplete combustion, the concentrations of tar and carbon monoxide can be higher than in typical cigarette smoke. The NCI's cigar fact sheet likewise notes: fermentation produces high nitrosamine levels, and per gram of tobacco, carcinogenic tar in cigar smoke can be greater.
In January 2024, during an indoor trial session in the north (a poorly ventilated mahjong room), I felt more directly how "temperature/combustion state" rewrites sensation: with the same cigar, when I drew too hard and carbon built up on the stub, the smoke instantly turned bitter and hot, and the nasal passage felt as if it had been plastered with chili powder; slowing down to an even smolder rounded the smoke back out. What rounds out is the taste, not the toxicant list. **Slow burning, long sessions, incomplete combustion** make it easy to misread "smooth" as "clean."
As for the magnitude of temperature, results vary a lot across experimental conditions, and I will not fabricate a precise myth of "cigars definitely burn at such-and-such degrees." The conclusion that matters for exposure: **combustion chemistry is jointly set by oxygen supply, tobacco density, wrapper permeability, and peak draw flow**; the cigar path overall leans toward "larger tobacco column + less filtration + longer exposure time," and the smoke-toxicity concentration is not "gentler" than cigarettes.
Inhalation Method: Mouth-Drawing Is Not Zero Exposure; Lung-Drawing Turns the Risk Profile Toward Cigarettes
This is the layer with the deepest misunderstanding.
Traditional cigar etiquette encourages holding the smoke in the mouth and exhaling rather than deep inhaling. Epidemiologically, exclusive cigar users average shallower inhalation than cigarette users. Hence folk logic auto-generates: "didn't go into the lungs ≈ basically fine."
The real picture is three-layered:
**1) Mouth-drawing still carries systemic nicotine and a local carcinogen load.** The oral and nasal mucosae can absorb nicotine quickly. The HPA's popular comparison: a single cigar can be on the order of about a pack of cigarettes — depending on the cigar's size (small cigars, cigarillos, and large premium cigars differ by an order of magnitude) and on how much and how long you actually smoked. Treating it as an exact conversion formula would mislead, but as a **magnitude warning** it holds: you think you are "sipping," and the system's intake can be far from shallow.
**2) Upper respiratory–digestive cancers do not require "lung-drawing" to occur.** The 2015 analysis by Chang et al. in *BMC Public Health* shows: cigar users have higher cancer-related mortality risk than never-smokers; the more smoked and the deeper the inhalation, the higher the risk; **even without inhaling the smoke into the lungs**, mortality risk for cancers of the mouth, ear, nose, throat, and upper aerodigestive mucosa is still higher than for never-smokers. Associations with the esophagus and oral cavity recur across multiple reviews and cohorts. The exposure site shifts from the alveoli to the mucosal surface; the risk is not cancelled — it is just that **the weight shifts across the disease spectrum**.
**3) People with a cigarette history can rarely really "mouth-draw only."** What happened later that night at the bar was typical: after mouth-drawing for about twenty minutes, his craving rose and he unconsciously took two obvious lung draws — shoulders dropping, smoke going deeper, an immediate cough. A man who once smoked 15 cigarettes a day has motor memory. The literature also notes: former cigarette smokers who switch to cigars tend to inhale; when used daily with inhalation, the overall exposure to harmful and potentially harmful constituents (HPHC) can approach that of daily cigarette users (similar statements appear in the NCBI assessment of premium-cigar health effects).
Biomarkers nail "I feel fine" to the page. In the 2014 study by Chen et al. in *Cancer Epidemiology, Biomarkers & Prevention*: people who had smoked at least one cigar in the past five days had urinary nitrosamine-related carcinogen markers about **19 times** those of non-smokers; serum cotinine was about **92 times**; with dual cigar-plus-cigarette use, the figures could rise to roughly **191-fold** and **1515-fold**, respectively. The numbers shift with population and assay, but the direction is consistent — **occasional, mouth-drawn, soft-feeling are not clean inside**.
My personal view: the inhalation method decides **how risk is painted on the body map**, not the master switch. Mouth-drawing leans toward mouth—throat—esophagus; lung-drawing pulls the lung and cardiovascular package back to full; dual use is nearly opening two input channels for yourself.
Where "Milder" Comes From: Neuroscience and Context, Not Toxicology Discounts
Break the sensation down; "mild" usually comes from these layers stacking:
- **Different nicotine kinetics.** Cigarette lung-draws hit the brain in seconds with a steep peak; cigar mouth-draws absorb through the mucosa with a blunter curve. The brain thinks "not as explosive," which says nothing about a lower 24-hour area under the curve (AUC).
- **Frequency illusion.** Many people smoke 20 cigarettes a day but only 1–2 cigars on weekends. Weekly exposure can temporarily drop, and they mistake "smoking less" for "this product is safer." Product properties and usage frequency get conflated.
- **Ritual and identity.** The decanter, the cap cutter, the lighter, the slow talk — these alone reduce anxiety and raise subjective comfort. Comfort is a psychological variable, not milligrams of tar.
- **Site shift of irritation.** The cough receptors of the lung and trachea are not bombarded the same way, so people assume less harm; the tongue and gums are quietly paying the bill.
In 2021, at a cigar bar in Chengdu, I asked a regular who smoked two medium cigars a day and had quit cigarettes for three years. His gum recession and bad breath were the most obvious at the table, yet he insisted: "My lungs are great; the X-ray is clean." A temporarily quiet lung image says nothing about the cumulative damage to the oral mucosa, nor about cardiovascular and tumor risks over the next decade. **Using the short-term sensation of one organ to deny the long-term exposure of many organs is the most common self-soothing.**
Folding the Three Differences into One "Exposure Ledger"
If you will remember one table, remember it like this:

The HPA and clinicians are actually highly aligned: **a cigar is not a quitting tool, nor a "safer cigarette."** Quitting cigarettes by switching to cigars is exchanging one delivery method of burning tobacco; through oral absorption and longer sessions, it may sustain nicotine dependence, and dual use can push nitrosamine and cotinine markers to even higher ranges.
My Conclusion and a Readable Way to Act
By this point, my stance does not need soft packaging:
The "mildness" of cigars is mostly a sensation manufactured by draw dynamics, irritation sites, and use context; the no-filter structure, more incomplete combustion, fermentation-related chemistry, and the mouth-draw/lung-draw behavior differences change the site, time structure, and toxicant concentration profile of exposure — they do not erase the harm from the ledger.
If you are using cigars to "bridge withdrawal," do at least three real things — don't go on feeling:
- **Count by session, not by number of sticks, to flatter yourself.** Record start time, end time, whether lung-draws occurred, and next-day breath and throat state. A 35–90 minute combustion session is not "a small taste."
- **Beware dual use.** Cigarette + cigar is one of the ugliest marker combinations; during withdrawal you are especially prone to double-dipping.
- **Treat upper-digestive symptoms as signals, not as "getting overheated."** Repeated mouth ulcers, bleeding gums, hoarseness, and swallowing discomfort are not accessories of cigar culture — they are the mucosa sounding the alarm.
Back to the Xuhui terrace: my friend found it mild that night; the next day at noon he told me his nose was dry, the tip of his tongue numb, and even coffee tasted flatter. That rebound in exhaled CO was just the one line most easily caught by an instrument. The other lines — TSNAs, mucosal dose, session length — are written in slower places, but they are still booked.
Sensation can lie; combustion chemistry and behavior patterns mostly don't. A cigar can be a personal choice made with full knowledge of the risks, but it is not a free pass for harm reduction, and it is not a replacement therapy for quitting smoking.
